If you need a Passport, below are some links about renewing, obtaining, and/or answering any other questions you might have.
If you don't have one, I do know that you have to fill out an application Form DS-11 but DO NOT SIGN until you are instructed to do so at the Acceptance Facility. Go to the Post Office and bring the Application DS-11, a certified birth certificate, a photo ID, 2 recent 2x2 in. color portrait photographs, and cash or credit card (fees are about $100).
